Create a Survey
Feedback is key to growing and improving your relationship with patients. Patients—like all of us—love the chance to be heard!
2.) Name your survey and click "Create Survey". This name will not be seen by the recipients.
1.) Under the "Projects" tab, click "Surveys" then click "New Survey".
4.) To add a question, click "Question 1" on the left. Hover over the question text and select "Click to edit".
3.) This is the initial message your patients will receive. Add text and images here that will prompt them to take the survey.
6.) To add more questions, click "Add Question +" to the left.
5.) Edit your question type, preamble, question, and answers. Click "Update".
8.) Enter your "From Name" (typically the hospital name) and the "Contact Email". Click "Save".
7.) After you have created all your survey questions, click "Settings" at the top right.
